New York State Office for the Aging (NYSOFA) is made up of a network of 59 local Offices and organizations which offer home- and community-based programs. They provide a well-planned, coordinated package of in-home and assistive services designed to aid and supplement informal care.

The Solution

CareDirector works in partnership with Peer Place to provide a state-wide IT system to replace the numerous indepdendant applications managed by local AREA Agencies. Including data collection, reports and assessments. CareDirector has updated NYSOFA’S vendor hosted NY Connects website and Resource Directory (https://www.nyconnecta.ny.gov).
